As we begin implementing learning labs next week, we would greatly appreciate if you can complete the form attached. This form is a district requirement and must be completed before students can enter the building. Please make sure that you are completing the form for each individual child in your household. After you have completed the commitment form, it is expected that parents please answer the screener checklist questions prior to bringing your child to school on that day.
